- Location of your emergency - Your name, address, and telephone number appear on the computer terminal when you call 9-1-1, but this information must be verified for your sake and that of the patient
- Nature of your emergency
- Your telephone number - We need the number from which you are calling in case the connection is lost or we need to contact you again
- What is wrong with the patient
- What is the age of the patient - Age is one determining factor for the type of response and type of treatment required (an infant choking would require a different response than an older patient)
- The gender of the patient
- Is the patient conscious or not?
- Is the patient breathing?
